Release of Liability Arts, Wellness, and Engagement Programming (AWE)

Waiver Details

I acknowledge that my choice to participate in any of the Arts, Wellness and Engagement (AWE) Programming Activities/Services provided by Jewish Family Service of Los Angeles (JFS) is voluntary.

I understand that precaution is taken to mitigate any hazards and prevent any injuries. However, in the event of any injury to myself, and in consideration of my participation in any of the AWE Programming Activities/Services, I hereby release and hold harmless JFS, its employees, contractors, volunteers, officers and directors from any liability for damages or claims for damages for personal injury, including accidental death, as well as from claims for property damage which may arise in connection with any of the AWE Programming Activities/Services, unless such claim is the result of JFS’ gross negligence.

I understand that it is my responsibility to check with my doctor to obtain clear guidelines and instructions before participating in any AWE exercise classes/activities that involve physical exertion and/or if my health status changes. I will alert JFS staff immediately if I experience any unusual symptoms during or following exercise at any JFS Fitness center and/or AWE Programming Activities/Services.

I understand that if I participate in AWE Programming, videos or photos may be taken to be used by Jewish Family Service of Los Angeles for their social media, newsletters, press and other collateral materials.

I understand that I have the right to refuse permission and that this will in no way interfere with my right to participate in AWE Programming.
